The basics for treating tooth decay have changed little since the father of modern dentistry Dr. G.V. Black developed them in the early 20th Century. Even though technical advances have streamlined treatment, our objectives are the same: remove any decayed material, prepare the cavity and then fill it.
This approach has endured because it works—dentists practicing it have preserved billions of teeth. But it has had one principle drawback: we often lose healthy tooth structure while removing decay. Although we preserve the tooth, its overall structure may be weaker.
But thanks to recent diagnostic and treatment advances we’re now preserving more of the tooth structure during treatment than ever before. On the diagnostic front enhanced x-ray technology and new magnification techniques are helping us find decay earlier when there’s less damaged material to remove and less risk to healthy structure.
Treating cavities has likewise improved with the increased use of air abrasion, an alternative to drilling. Emitting a concentrated stream of fine abrasive particles, air abrasion is mostly limited to treating small cavities. Even so, dentists using it say they’re removing less healthy tooth structure than with drilling.
While these current advances have already had a noticeable impact on decay treatment, there’s more to come. One in particular could dwarf every other advance with its impact: a tooth repairing itself through dentin regeneration.
This futuristic idea stems from a discovery by researchers at King’s College, London experimenting with Tideglusib, a medication for treating Alzheimer’s disease. The researchers placed tiny sponges soaked with the drug into holes drilled into mouse teeth. After a few weeks the holes had filled with dentin, produced by the teeth themselves.
Dentin regeneration isn’t new, but methods to date haven’t been able to produce enough dentin to repair a typical cavity. Tideglusib has proven more promising, and it’s already being used in clinical trials. If its development continues to progress, patients’ teeth may one day repair their own cavities without a filling.
Dr. Black’s enduring concepts continue to define tooth decay treatment. But developments now and on the horizon are transforming how we treat this disease in ways the father of modern dentistry couldn’t imagine.
It's a big transition when your child enters college — for both of you. You may find “cutting the apron strings” a little rocky at times.
But like most parents, you'll soon condense what you still want your college kid to do down to a few major habits and choices. Be sure to keep health, diet and lifestyle choices on that list, areas which could have the most effect on their long-term health and well-being.
That should include dental care. Hopefully, they've already developed good hygiene habits like daily brushing and flossing and regular dental visits. But, on their own now, they're faced with other choices that could affect their dental health.
For example, eating a balanced, nutritious diet is necessary for a healthy mouth. That includes limiting sugar intake, especially when snacking. Disease-causing oral bacteria thrive on carbohydrates like sugar. These bacteria also secrete acid, which at consistently high levels can erode tooth enamel.
Tobacco smoking and excessive alcohol affect teeth and gums because both can inhibit secretion of saliva. Besides containing antibodies that fight infection, saliva also neutralizes mouth acid. A dry mouth caused by these habits, could put their mouth at higher risk for disease.
Your college student might also be influenced by the fashion of their peers to display piercings. Mouth piercings with lip or tongue hardware in particular can damage teeth. The constant movement and friction erodes enamel or may even cause a tooth fracture. If possible, try to steer them to self-expression that poses less risk to their dental health.
There's one other area that, believe it or not, could impact dental health: sex. While each family handles this particular subject differently, be sure your child knows that some forms of sexual activity increase the risk for contracting the human papilloma virus (HPV16). Among its many destructive outcomes, HPV16 profoundly raises the risk of oral cancer, a rare but deadly disease with a poor survival rate.
Going from home to college is a big step for a young person — and their parents. As a parent, you can help steer them to practice good habits and make wise choices that will protect their lives and health and, in particular, their teeth and gums.

There’s been a growing awareness about the effects of gluten, a protein found in grains like wheat, rye and sometimes oats, on certain people. An estimated 1 in 133 Americans have Celiac Disease (CD), a gluten-related disorder that causes the body’s immune system to work against itself. And if you have CD, you could eventually face dental problems like enamel pitting and erosion.
When a person with CD consumes gluten, their immune system mistakenly identifies the protein as malicious and attacks it. The attack occurs in the membranes that line the digestive system, which in the process destroys cilia, tiny hair-like structures that aid in food absorption. This disrupts the body’s normal absorption of nutrients, which can lead to a number of systemic conditions including intestinal cancer.
Because of the lack of nutrients, your teeth’s enamel may develop defects. You may begin to see dull spots or pitting, or chalky grooves in its normally shiny surface: this is a sign you’ve lost surface enamel crystals (decalcification). You may also be more susceptible to outbreaks of aphthous ulcers (canker sores).
Because symptoms can be misdiagnosed or go unnoticed, it may be years before you know you have CD. You can, however, get a definitive diagnosis through a blood test for gluten antibodies, which is then confirmed with a biopsy of a tissue specimen from the intestine.
While there’s ongoing research for CD-related medication, there’s currently only one recognized treatment for it — remove gluten from your diet. This is much harder than it sounds, and requires knowing what you can and can’t eat, along with strict monitoring of food package labeling. Thankfully, the world is becoming better educated in this respect as more food manufacturers are clearly labeling products containing gluten and restaurants are providing gluten-free menu options.
Once you have dietary controls in place, your dental issues can be treated as any other person, with one exception: none of the products used in treatment like polishing paste or fluoride gels should contain gluten, and must be verified before using.
CD is a serious condition that could even become life-threatening. Knowing you or someone in your family has it will help you protect both your overall health and your teeth.
